# Capture HTML Form Submission (Predesigned) — Forms

Botize method — `btzforms.webhook_html_form_predesigned_create` (trigger).

Triggers when a new HTML form submission is received using one of the predesigned templates.

## Fields you set

The field's internal name, its type and the values it accepts. These fields accept `{{...}}` tags from earlier steps.

- `template` *(string, one of: "button_only" = Submit button only; "text_button" = Text area and submit button)* — Template

## Output variables

- `{{task_trigger_at}}` *(date)* — Trigger date/time. Date and time when the task is executed.
Format: Unix epoch time (e.g., 1657890000)
